Timothy Sloan PA-C

Tim has extensive experience as a Cardiology PA (Physician Assistant) and as a registered nurse. After completing his education and training to become a certified Physician Assistant at University of North Dakota in 2002, Tim has been active in caring for cardiac and vascular patients as a PA for nearly a decade. He is an integral part of Same Day HeartCare.

Danielle Martin, NP-C

Danielle Martin graduated with a Bachelor of Science in Nursing from Michigan State University. Through working as a nurse on an Intermediate Intensive Care Unit (IICU) for the past eight years she gained an incredible amount of knowledge through innumerable experiences working with critically-ill patients. She completed her Masters of Science in Nursing as an Acute Care Adult Gerontology Nurse Practitioner and her passion for cardiology has drawn her to join the practice.

Hannah Jang, NP-C

Hannah Jang is a board certified family nurse practitioner. She has extensive experience as a nurse practitioner and as a Registered Nurse over 15 years. She received her bachelor of science in nursing at Shamyook University in Korea and her master of science as a family nurse practitioner from the College of New Jersey. Her passion is in primary care and cardiology.

Jared Graves, PA-C

Jared Graves is a nationally certified physician assistant. He graduated from the University of Toledo with a Master of Science in Biomedical Sciences in 2022. Previously, he graduated from the University of Michigan - Flint with a Bachelor’s in science. Jared has experience as a Firefighter and Emergency Medical Technician - Basic. He enjoys watching movies and spending time with his significant other and family.
